COVID-19 | Player Declaration to Complete Before Return

Before returning to rugby league activity, all players must complete the Player Declaration Form. Those who completed the form in 2020 do NOT have to complete it again for 2021.

All players must opt IN by completing this form to acknowledge the risks involved and agreeing to the code of conduct. Parents & guardians should complete and sign the form for junior players.

Failure to complete the declaration will mean players are NOT allowed to take part in any activity.

Download the below form and complete it and return it to info@cramlingtonrockets.co.uk by 6pm Wednesday 31st March 2021. Please note you do NOT need to complete this form if you submitted a 2020 version.
